DATA SUBJECT APPLICATION FORM
Law No. 6698 on the Protection of Personal Data (the “Law”) grants data subjects the right to make certain requests regarding the processing of their personal data.
This Data Subject Application Form has been prepared so that applications made by data subjects to the data controller, Motto Danışmanlık Mühendislik ve Teknoloji Şirketi (the “Company”), regarding these rights under Article 11 of the Law can be assessed and resolved effectively and comprehensively.

- How to Apply
You may submit your requests concerning the application of the Law to our Company, as data controller,
- by applying in person at “Orhanlı mahallesi Derviş caddesi No:24 GAT Plaza K:2 Tuzla, Istanbul” with a signed copy of this Data Subject Application Form; or
- by sending this Data Subject Application Form to our Company’s e-mail address info@motto-tr.com via your registered e-mail address (KEP), a secure electronic signature, a mobile signature, or an e-mail address previously notified by the data subject to the data controller and registered in the data controller’s system.
Your applications will be concluded free of charge as soon as possible depending on the nature of your request, and within thirty days at the latest; however, if the process involves an additional cost, a fee may be charged according to the tariff set by the Personal Data Protection Board.
